United Cerebral Palsy and Area on Aging

Our services that we provide at From The Heart are eligible to be free to you or your loved ones. The reason that these services can be free is due to our company having a license with the state of Pennsylvania for a Home Care Agency Facility.  Pennsylvania has wavier programs available for individuals in need of  home health care.  To find out more about these types of services contact our office and we will direct you to proper intake number either Maxiums or your local Area on Aging.

When you contact either of these entities, you will go through a process that will determine your eligibility for a waiver. The main criteria for any waiver is how great you or your loved one’s need is. The greater the need is, the more likely you will be eligible to be on a waiver program.

Food Programs for Seniors and the Disabled

There are many programs and businesses out in your area that provide hot or cold meals at little or no cost to you. The list below is of these programs that you may contact to help feed you and your family.

Transportation Services

Allegheny County

  • Access Transportation Services
    ACCESS is door-to-door, advance reservation, shared ride transportation provided throughout Allegheny County and areas 1.5 miles outside Allegheny County. They service primary senior citizens and persons with disabilities. Service is provided from 6:00 am to midnight, seven days a week.   • Medical Assistance Transportation Program (MATP)
    MATP provides residents in Allegheny County free, non-emergency medical transportation to and from MA billable services. Such appointments include dentists, doctors, pharmacies,  mental health counseling, and drug and alcohol counseling. To be eligible, you must be eligible and receiving Medical Assistance, be a resident of allegheny county, and the necessary paperwork must be filled out before you can use these services. Westmoreland County

  • Senior Free Transit Program
    The transit authority of Westmoreland County provides seniors over the age of 65 free bus transportation. To obtain a Senior Transit ID Card, you must apply in person at the Transit center in downtown Greensburg. Proper identification must be shown when applying.
    The following is acceptable: Driver’s License, PA Identification Card, Birth
    Certificate, Baptismal Certificate, Passport, Military Discharge Papers, or PACE Prescription Card.
  • Reduced Fare Transit Program
    This program enables persons with disabilities to ride a public bus for one-half of the regular fare. To apply for this program, please call  1-800-221-9282 to request an application. The application must be completed by a healthcare provider prior to you returning it to the Transit Center. Individuals who are under the age of 65 and possess and Medicare card will qualify for a Reduced Fare card without the verification of a healthcare provider.
  • Medical Assistance Transportation Program (MATP)
    This program is funded through the Department of Public Welfare and provides free transportation to medical appointments for residents who possess or request or eligible for a Welfare Medical Card. Transportation can be provided by three options: mileage reimbursement for use of a private vehicle, public bus transportation, or taxi service.
    You must register by calling the Medical Assistance Transportation Department at  1-800-242-2706. The application must be completed by both the applicant and by a health care provider and returned to Westmoreland Transit. Applications are reviewed by an independent agency and eligibility is based on the diagnosed disability.
    Once the application has been approved, a trip can be scheduled by calling 724-832-2211 at least one day in advance. The fee for this service is twice the normal bus fare to the same destination.
  • Americans With Disabilities (ADA) Complementary Paratransit Program
    The ADA Complementary Paratransit Program provides transportation for individuals whose disability prevents them from using a public bus. The service is provided by taxi and “mirrors” the public bus service. It is available during the same hours and to the same destinations that are available through the public bus service.
    To apply for this service, call the Transit Center at 1-800-221-9282 and request an ADA Complementary Paratransit Application. The application must be completed by both the applicant and by a health care provider and returned to Westmoreland Transit. Applications are reviewed by an independent agency and eligibility is based on the diagnosed disability.
    Once the application has been approved, a trip can be scheduled by calling 724-832-2211 at least one day in advance. The fee for this service is twice the normal bus fare to the same destination.
  • Persons with Disabilities Program (PwD)
    The PwD Program provides transportation for individuals who have a disability to areas not served by existing transportation services.  This program is funded through PennDOT, and provides a reduced fare shared-ride service for disabled persons between the ages of 18 and 64 who live in Westmoreland County.  Hours of service vary depending on location.
    To apply for this service, call the Paratransit Department at 724-832-2211 and request a PwD application.  The disability must be verified by a health care provider.
    Once the application has been approved, a trip can be scheduled by calling 724-832-2211 at least one day in advance.  Fees for this vary depending on distance traveled. Exact costs will be provided when the ride is scheduled.  Riders must pay cash to the taxi driver on the day of the trip.
    This program can be used for any purpose, but only applies to trips that are not covered by any other transportation program.
